Dear Brixham Youth Coach/Parent,

Re: CRB Disclosure Application Form


As you may be aware the Rugby Football Union ("RFU") has made significant progress on child protection issues and has developed a Child Protection Policy. This Policy identifies the responsibilities to be followed by the RFU, the Constituent Body and the individual Rugby Clubs. Brixham Youth Rugby Football Club has in place a Child Protection Policy that is in line with the recommendations made by the RFU and as part of this policy it requires all members who have regular supervisory contact with children or management responsibility for those working with young people to undertake a Criminal Records Bureau disclosure.

The Criminal Records Bureau ("CRB"), an executive agency of the Home Office, has prepared a Disclosure Application Form to help employers and volunteering organisations make informed recruitment decisions to protect vulnerable members of society. By filling in this application form and participating in disclosure, you will be contributing towards improving standards, both in our own environment and in society as a whole.

Initially all mini and youth coaches/parent helpers plus the Committee of Brixham Youth Rugby Football Club will be required to complete a Disclosure Application Form which will be sent to the RFU. Refusal or failure to submit a completed form may result in that person being excluded from the Club. This may seem a touch draconian but please remember the intentions of the RFU and Brixham Youth Rugby Football Club. The Club has to provide a duty of care to young people and has a responsibility to protect them from harm.

I will be dealing with the administrative arrangements for the completion of the Forms, including the liaison with the Devon Area Welfare Officer and the RFU.

Otherwise please complete Sections A to D and Section H of the form enclosed.
The form must be completed in Block capitals and Black ink

To complete the form you will need:
· Details from your birth certificate
· Your National Insurance Number
· Full postal address of homes where you have lived during the past five years.
· A preferred contact telephone number
· You are also asked to show certain original documents. As a minimum requirement you will need to show your passport or photo driving licence, together with proof of current address (recent utility bill, bank statement etc.)

To assist with the completion please note the following.

· In Section B
13. Position applied for is Volunteer Rugby Coach

· Section E Additional information. No longer requires completion.
· Section F Referee details. No longer requires completion.
· Section G payment. Leave blank
· Section X leave blank. To be completed by the Clubs Child Protection Officer
· Section Y leave blank. This section is completed by the RFU.

When completed the form should be provided to me in person together with the original documents as advised above. I will complete Section X of the Form and the RFU's disclosure of information form for each applicant and hand back the original documents.
I will be at the Club most Sundays for Form authorisation.
